Why does the first-year revenue assume clients build evenly?

Because an app launched in January does not have its December client count all year. The sheet averages the target across the months after the first sale, which is a plain estimate stated in the method; if your pipeline is front-loaded, enter fewer months to the first client.

What belongs in the setup cost?

The vendor's setup fee, your designer's work on the branding and the screens, the app store listing work and any content the app launches with. Enter it as one figure; the sheet recovers it from the first-year net.

Is the per-client fee the same as the wholesale price on the margin worksheet?

Usually yes: the vendor's charge per client account a month. App builders often charge a larger monthly fee for the branded app and a smaller per-client fee, which is why this sheet asks for both.
